Necesidades psicosociales de residentes y familiares en centros residenciales: perspectiva del equipo asistencial y factores asociados
Psychosocial needs of residents and family members in residential care facilities: perspective of the care team and associated factors

RESUMEN

Introducción: Poco se sabe en nuestro país sobre los procesos de fin de vida y cuidados paliativos en centros residenciales (CR). El objetivo de este estudio es describir las necesidades de atención psicosocial de los residentes y sus familiares según el equipo del CR y analizar factores asociados.
Material y métodos: Estudio descriptivo retrospectivo. Por muestreo no probabilístico de conveniencia se seleccionaron 4 CR de Barcelona. Se realizaron dos reuniones con cada equipo. En estas reuniones se recogieron las respuestas a la pregunta “sorpresa” del instrumento NECPAL CCOMS-ICO© y la percepción de si residentes o familiares pudieran beneficiarse de intervención de atención psicosocial. En un cuestionario ad hoc se clasificaron los dos principales motivos por los que sería necesaria su intervención. Se registraron también los siguientes datos extraídos de la historia clínica compartida de Catalunya y del CR: demográficos, complejidad, estado cognitivo (Short Portable Mental State Questionarie), emocional (Geriatric Depression Scale), si tenían el documento de voluntades anticipadas y si recibían visitas familiares.
Resultados: Se analizaron los datos de 375 residentes, con una edad media de 85,15 ± 9,57 años, el 71,2 % mujeres. El 43,47 % presentaban necesidades de atención paliativa según respuesta positiva a la pregunta “sorpresa” y el 54,4 % de los residentes y el 16 % de sus familiares eran percibidos como susceptibles de atención psicosocial. En los residentes se encontraron diferencias estadísticamente significativas de atención psicosocial por estado cognitivo valorado con el Short Portable Mental State Questionnarie (media errores [sí] = 7,05 ± 2,92 vs. Media errores [no] = 5,72 ± 3,26); p < 0,001), y emocional mediante la Geriatric Depression Scale de Yesavage (puntuación media [sí] = 6,95 ± 2,69 vs. puntuación media [no] = 5,36 ± 2,67; p = 0,001).
Conclusiones: La alta prevalencia de necesidades de atención psicosocial en residentes, y en menor medida en familiares, junto con su asociación con el deterioro cognitivo y emocional, refuerza la importancia de su identificación sistemática y abordaje integral en los CR.



ABSTRACT

Introduction: In our country, little is known about end-of-life processes and palliative care in Nursing Homes (NH). The objective of this study is to describe the psychosocial care needs of residents and their families, according to the NH team, and to analyze associated factors.
Material and methods: Retrospective descriptive study. Four NH were selected through non-probabilistic convenience sampling. The psychologist conducted two meetings with each team. In these meetings, responses to the “surprise question” from the NECPAL CCOMS-ICO© instrument were collected, along with the perception of whether any residents or family members could benefit from psychosocial care interventions. The two main reasons for requiring intervention were classified using an ad hoc questionnaire. The psychologist also recorded data extracted from the shared clinical history of Catalonia and the NH, including demographic variables, complexity, cognitive status (Short Portable Mental State Questionnaire), emotional (Geriatric Depression Scale), whether an advance directive document was available, and whether residents received family visits.
Results: Data from 375 residents were analyzed, with a mean age of 85.15 ± 9.57 years, 71.2 % women. Of these, 43.47 % presented palliative care needs according to positive response to the ‘surprise question’, and 54.4 % of residents and 16 % of their relatives were perceived as requiring psychosocial care. Statistically significant differences in psychosocial care needs were found among residents according to cognitive status assessed with the Short Portable Mental State Questionnaire (Mean errors [yes] = 7,05 ± 2,92 vs. Mean errors [no] = 5,72 ± 3,26) and emotional status assessed with the Geriatric Depression Scale (Yesavage) (Mean Score [yes] = 6,95 ± 2,69 vs. Mean score [no] = 5,36 ± 2,67; p = 0,001).
Conclusions: The high prevalence of psychosocial care needs among residents, and to a lesser extent among their relatives, together with their association with cognitive and emotional impairment, highlights the importance of systematic identification and comprehensive management of these needs in nursing homes.
