RESUMEN

Antecedentes y objetivo: En España y en el mundo, el cáncer infantil es una de las principales causas de muerte durante esta etapa de la vida. Aunque en la niñez son muchas las neoplasias que se curan completamente, un alto porcentaje de casos siguen siendo mortales. La mayoría de los niños que fallecen por cáncer pasan por una etapa previa de estado terminal susceptible de necesitar cuidados paliativos pediátricos. Globalmente existe una gran heterogeneidad en la dispensación de estos cuidados según el lugar de residencia. Es importante conocer las experiencias y preferencias, tanto de los propios niños como de sus familiares, para garantizar un cuidado excelente en esta etapa de la vida. Nos planteamos los siguientes objetivos: describir las experiencias y recomendaciones de los padres ante el actual sistema de cuidados al final de la vida que experimentaron sus hijos; identificar los aspectos que favorecían o dificultaban el establecimiento de una relación terapéutica eficaz; analizar el rol tanto de los padres como de los profesionales sanitarios, especialmente los profesionales de enfermería.
Materiales y métodos: Se llevó a cabo una metasíntesis cualitativa de los 16 artículos identificados a través de una revisión bibliográfi ca sistemática de la literatura a través de varias bases de datos: PubMed, Science Direct y Web of Science.
Resultados: El análisis cualitativo de la información resultó en las siguientes categorías: 1) lo que el equipo sanitario puede/debe hacer; 2) familia: unidad de cuidado y demandante de cuidado; 3) comunicación como pieza clave de la relación; 4) luchando contra la enfermedad mediante el conocimiento; 5) el niño como centro del cuidado, y 6) el fallecimiento es solo el principio.
Conclusiones: El presente trabajo pone de manifi esto la importancia del establecimiento de una relación de confianza entre los miembros del equipo de CPP y los niños y sus familias. Una actitud empática, la participación activa de los padres en el cuidado de sus hijos, y un proceso de comunicación abierto y fluido entre los profesionales sanitarios y los padres son esenciales para mejorar la calidad del cuidado. Por otra parte, es necesario mejorar la formación especializada del personal sanitario responsable del cuidado de los niños con cáncer. Existe una escasez de estudios liderados por profesionales de enfermería en este campo, así como una clara falta de atención al rol y responsabilidades de estos durante el proceso de atención al niño y su familia al final de la vida.



ABSTRACT

Background and aim: Worldwide, childhood cancer is one of the leading causes of death during childhood. Although many childhood malignancies can be cured, a high percentage of these children will end up dying of cancer. Most children who die from cancer undergo a previous end-of-life stage, which will require pediatric palliative care (PPC). Globally, there is great heterogeneity in the provision and dispensation of PPC. It is thus important to know the experiences and preferences of these children and their families in order to guarantee excellent care throughout this stage of their lives. We planned to achieve the following aims: to describe the experiences and recommendations of parents with terminally ill children receiving PPC; to identify the aspects that favor or hinder the establishment of a therapeutic relationship between health care professionals and children and their families; to investigate the role of parents throughout this period, and to analyze the role of the healthcare professionals involved, especially the nurses.
Material and methods: We undertook a qualitative metasynthesis of 16 scientific articles identified following a systematic review of the literature in the following databases: PubMed, Science Direct, and Web of Science.
Results: A qualitative analysis of the primary sources was carried out by grouping the information into the following categories: 1) what the healthcare team can or should do; 2) family: a care giving and care demanding unit; 3) communication as a key part of the relationship; 4) fighting the disease through knowledge; 5) the child as the focus of care; and 6) death is only the beginning.
Conclusions: This article brings to light the importance of establishing a trust relationship between the members of the PPC team and these children and their families. Maintaining an empathic attitude, involving the parents in their children’s care, and keeping an open, fluid communication between healthcare professionals and parents are essential to improve quality of care. In addition, it is necessary to provide all health professionals involved in the care of these children and their families with specialized training in PPC. There is a lack of studies being led or undertaken by nurses in this area, as well as a lack of attention to the role and responsibilities of these professionals during end-of-life care for these children and their families.
